When asked who prescribed this medication click the dropdown and select 'Add New Provider'. Enter your provider's Zip Code and Full Name and in most cases you will be able to find your provider.

If you are still running into issues there is an option to enter the information yourself. Click 'Add Custom' located in the bottom right of the pop-up and you will be prompted to enter your provider's First Name, Last Name, Zip Code (of their clinic), Phone Number, and Fax Number (optional). You can then click 'ADD & SAVE' and continue on with the ordering process.
